SERVICIOS DE COORDINACIÓN DE APOYO

 

Los servicios de coordinación de apoyo son servicios que ayudan a los participantes a obtener acceso a los programas necesarios y los servicios del plan estatal, como asistencia con beneficios médicos, servicios sociales, educativos y de otro tipo. El Coordinador de Soporte es responsable de desarrollar y mantener el Plan de Servicio Individualizado con el  participante, su familia (si corresponde) y otros miembros del equipo designados por el participante. El rol del Coordinador de apoyo consta de 4 funciones generales: descubrimiento individual, desarrollo de planes, coordinación de servicios y monitoreo.

Simply print and fill out the form below, and send it by mail or e-mail to one of the addresses listed on the form. After a few weeks, DDD will notify Caring For Families Social Services about the new individual assigned to CFFSS. We will call you within 24 hrs of receiving the notification from DDD to set up an intake appointment and begin the process.

Select Caring For Families Social Services as your loved one's Support Coordination Agency

by filling out the Support Coordination Agency Selection Form and submitting it to DDD.

 

IMPORTANT: Requests to be assigned to a Support Coordination Agency are made by DDD at the beginning of the month. Submit your request to DDD as soon as possible!​

Graduating high school?

If your child is graduating high school or aging out of the school system, start the DDD enrollment process as soon as possible in order to access adult services covered by the NJ Division of Developmental Disabilities right on time. To be determined eligible for services all applicants must complete either the DDD "short" or "full application".

  • When the individual is in their last year of receiving school-based services, the Support Coordination Agency Selection Form is completed and submitted.
